What this procedure decides
Resolve an access or equal-treatment barrier in a concrete public service.
The office examines usability, provider duties, confidentiality and representation. It agrees an accessible alternative before closing the underlying service as incomplete.
Acting in the right capacity
People and households
Residents, passengers, workers, learners, caregivers and authorised advocates. State whether you act for yourself, a dependant or a person who authorised your representation. Keep household and care records within the request’s actual purpose.
Companies and cooperatives
Suppliers, licensed operators, enterprises, worker-owned organisations and professional providers. Name the legal or cooperative operator, your authority to act and any supplier or utility interfaces. Commercial, technical and civic permissions remain separate findings.
Research and civic organisations
Laboratories, universities, observatories, associations, cultural organisations and public-interest networks. Name the accountable organisation, research or civic purpose, relevant ethics or protection review and the custodian who can authorise source access.
Public administrations
Local councils, regional offices, Union bodies and authorised public-service delegates. Name the originating power, receiving scope, authorised delegate, reserved decisions and the event at which the other office accepts a duty.
Prepare assessable evidence
Incident or barrier description
Identify the source custodian, version and purpose of incident or barrier description. The affected service, practical barrier and requested adjustment define the matter. An adjustment request is not a requirement to reveal unrestricted medical or personal information.
Service location
For service location, name the receiving scope and any outstanding acceptance. Explain a missing or disputed element instead of silently replacing it with an unrelated record.
Requested accommodation
For requested accommodation, provide the independently assessable finding or agreed alternative. State its conditions, expiry or material-change trigger; a favourable summary is not a substitute for the source.
Alternative evidence and a disputed source
Identify the missing element, its custodian and the alternative you can provide. Preserve conflicting versions and explain the difference. The receiving office states what it can establish and which question remains open.
Read the result before accepting its scope
An adjustment agreement, a provider review, an independent finding or a reasoned referral.
Track whether the alternative is usable in practice and who owns its maintenance or cost. Preserve the complaint route if the barrier returns.
Who can use this service
A person, representative or affected organisation encountering discrimination or a practical public-service barrier. The requested adjustment is considered before the underlying case is closed as incomplete.
A practical equality remedy and independent review route.
The applicable safeguard
An accessible alternative is part of the service, not a discretionary favor
Prepare a request
The online workflow supports residents and authorized representatives, operators, researchers and public delegates. A saved receipt stays on this browser. The PDF packet provides a fillable offline record.
The local register issues a receipt when the request is recorded. Published service clocks use receipt and verified-completeness events; numeric deadlines remain charter decisions in this fictional edition.
Any fee, exemption or numeric deadline must be published by the competent institution. This edition does not invent a currency or fee schedule.
