Local councils, regional offices, Union bodies and authorised public-service delegates. Name the originating power, receiving scope, authorised delegate, reserved decisions and the event at which the other office accepts a duty.
Identify the source custodian, version and purpose of record reference. The disputed entry, originating register and correction evidence establish the request. Standard identity evidence has an accessible alternative where necessary.
For correction evidence, name the receiving scope and any outstanding acceptance. Explain a missing or disputed element instead of silently replacing it with an unrelated record.
For identity verification basis, provide the independently assessable finding or agreed alternative. State its conditions, expiry or material-change trigger; a favourable summary is not a substitute for the source.
Identify the missing element, its custodian and the alternative you can provide. Preserve conflicting versions and explain the difference. The receiving office states what it can establish and which question remains open.

Append the effective event, reason and review route; do not silently delete the original source history.
