Identify the source custodian, version and purpose of prior learning. The training scope, supervisor competence and workplace agreement define duties. Safety and employment protection are not replaced by calling the participant a learner.
For host training agreement, name the receiving scope and any outstanding acceptance. Explain a missing or disputed element instead of silently replacing it with an unrelated record.
For safety training record, provide the independently assessable finding or agreed alternative. State its conditions, expiry or material-change trigger; a favourable summary is not a substitute for the source.
Identify the missing element, its custodian and the alternative you can provide. Preserve conflicting versions and explain the difference. The receiving office states what it can establish and which question remains open.
A supervised learning agreement, a revised placement scope or a reasoned finding on unmet conditions.
Completed competencies remain portable. A new supervisor records accepted work and the precise assessment still required.
