For travel arrangements, name the receiving scope and any outstanding acceptance. Explain a missing or disputed element instead of silently replacing it with an unrelated record.
For care and housing plan, provide the independently assessable finding or agreed alternative. State its conditions, expiry or material-change trigger; a favourable summary is not a substitute for the source.
Identify the missing element, its custodian and the alternative you can provide. Preserve conflicting versions and explain the difference. The receiving office states what it can establish and which question remains open.
